晚期乳腺癌化疗方案的探讨

目的　探讨米托蒽醌 ,5 FU ,大剂量的CF方案、NFL方案在治疗晚期发生转移的乳腺癌中的疗效及毒副作用。方法　 75例患者接受NFL方案治疗 ,米托蒽醌 12mg/m静推第 1天 ,CF 30 0mg静滴第 1～ 5天 ,2小时内滴完后静滴 5 FU 35 0mg/m第 1～ 5天。另外 75例接受CMF方案 ,CTX 6 0 0mg/m静推第 1天 ,MTX 40mg/m静推第 1天 ,5 FU 6 0 0mg/m静滴第 1天 ,均应用三周期以上。 结果　NFL的缓解率明显高于CMF方案的缓解率 (前者为 5 8.6 3 % ,后者为 30 .7% ,P <0 .0 5 ) ,中位生存期两者相似。毒副作用都能耐受。结论　NFL方案是治疗发生转移的晚期乳腺癌的一种较佳方案 ,对晚期乳腺癌有较高的缓解率和较低的毒副作用。

Chemotherapeutic regimen for patients with advanced breast carcinoma

Objective To evaluate the therapeutic effect and side effects of combination of mitoxantrone,5 fluorouracil,and high dose leucovorin in the treament of advanced breast carcinoma.Methods Seventy five patients were treated with NFL regimen:mitoxantrone 12mg/m 2iv on day 1,leucovorin 300mg iv bolus in 2 hours immediately followed by 5 fluorourcil 350mg/m 2 iv bolus on 1,2,3,4,and 5 Days.Seventy five patients received CMF:cyclophosphamide 600mg/m 2 iv on day 1,methotrexate 40mg/m 2 iv on day 1,and 5 fluorouracil 600mg/m 2 iv on day 1.Three therapeutic courses were given with an interval of 21 days.Results Patients treated with NFL had a higher response rate than patients with CMF(58.63% vs 30.7%),p<0.05.Median survival was similar for both groups.Both regimens were well tollerated.Conclusions NFL is a good regimen for patients with davanced breast carcinoma,which has a higher response rate and lower toxicities.
