手足口病37例临床分析

目的：探讨手足口病37例临床分析。方法：对2007-2008年我院门诊和住院部收治的37例手足口病的临床资料进行了回顾性分析，即对患者的性别、年龄、首发症状、皮疹形态、发展过程、部位，并发症辅助检查结果、疗效及死亡原因等进行分析。结果：37例患儿中11例（29．7％）发生在人员聚集单位，62．2％的皮疹与口腔粘膜疱疹同时出现，先为玫瑰色红斑或斑丘疹，24h后转为疱疹，好发于手掌、足底、指趾的屈面等表皮厚而容易受到摩擦的部位，在口腔、手掌同时出现皮疹者为23例，口腔、手掌、足底同时出现者为14例（37．8％；血常规检查：白细胞总数正常18例（48．4％），白细胞总数升高5例（13．0％），白细胞总数下降14例（37．8％）；淋巴细胞增高占23例（62．16％），中性粒细胞较低5例（13．51％），中性粒细胞增高8例（21．62％）。心电图检查：心电图异常11例，窦性心动过速8例（21．6％），心肌炎2例（8．11％）。肝功异常即单项转氨酶增高3例（8．1％）。胸片检查，支气管炎4例（10．8％），支气管肺炎3例（8．1％），合并扁桃体炎5例（16％），脑电图广泛异常1例（4％）。结论：本病系由病毒感染性疾病，呈自限性，经抗病毒及对症治疗，支持疗法后预后良好，很少有并发症，但重症患者可合并心肌炎及脑膜炎危及生命。预肪本病传播流行的关键是及时发现并隔离患儿。

clinical analysis of 37 cases with .hand-foot-mouth disease

Objective： To approach 37 clinical case with hand-foot-mouth disease. Method： To carry out retrospective analysis on 37 clinical cases with hand-foot-mouth disease in our hospital in 2007-2008, and to analyze the of sex, age of patients the first symptom, rasher, development process, complications, laboratory cases results, curative effect, death reason and so on. Results： In all of 37 cases, 11（29.7%） cases occurred at the personnel place, 26（62.2%） skin rashes and herpes in oral mucosa appeared in the meantime, at first it was rose erythema or maculopapule,and turned into bled after 24 hours, and ordinary at palm, foot bottom, point a bending of toe noodles etc. In 23 cases their skin rash appeared in the meantime in the mouth cavity and palm, 14（37.8%） examples appears in the meantime at the mouth cavity, palm and foot bottom; In blood routine inspection： The white cell amounts were normal in 18（48. 4%）, the white cell total amount went up in 5 （13. 0%）. The white cell total amount declined in 14（37. 8%）; Lymphoid cell went up in 23（62.16%）, neutral cell lower 5（13.51%）, neutral cell higher in 8（21.62%）. The electrocardiogram abnormaly were 11, sinus tachycardia 8（21.6 %）, and myocarditis 2 （8. 11%） abnormal . Liver function namely the higher single transaminase in 3（8.1%）. Thoracic film： there were 4 bronchitis （10.8%）, bronchopneumonia 3（8.1%）. Compliatey with 5 tonsillitis （16%） , electroencepha- logram extensively excrescent in 1（4%）. Conclusion.. This disease is come by the virus infection and have a limited course. After the antiviral therapy and supportive treatment, the prognosis of most patients is good, the complication is seldom seen, but grave patient could complic sufferer the disease, such as myocarditis and meningitis etc and also threat to life. To prevention this disease, the key just is discover patient.
