| 腹腔镜下中低位直肠癌保肛术技巧探讨 |

| 摘 要： | 目的总结腹腔镜下中低位直肠癌保肛术的手术经验,探讨相关技巧。方法回顾性分析67例腹腔镜下完成中低位直肠癌保肛术的临床资料。结果无死亡病例,术后随访,发生吻合口漏3例,吻合口狭窄2例,局部复发率为7.46%,癌转移率为13.43%,3年存活率为79.1%。结论腹腔镜下行中低位直肠癌保肛术可最大限度地保留肛门功能,降低术后复发率、病死率,但要遵循肿瘤根治原则和"直视、锐性、间隙、完整"的原则,并结合应用好双吻合器。

| Abstract: | Objective To research and discuss the laparoscopic technique of sphincter preserving surgery in low rectal cancer.Methods Retrospective analysis of clinical data of 67 cases of laparoscopic surgery in low rectal cancer was done.Results Post-operative follow-up:no death,3 cases of anastomotic leakage,2 cases of anastomotic stenosis,local recurrence rate 7.46%,metastasis rate 13.43%,3-year survival rate 79.1%.Conclusion The laparoscopic rectal cancer surgery can maximize the retention of anal function,reduce recurrence and mortality,while the radical principle and "direct vision,sharpening,gap,integrity" principle should be followed,combined with the application of double anastomat. |
