不同外科手术治疗分化型甲状腺癌的疗效观察

目的探讨不同外科治疗手段对于分化型甲状腺癌的疗效及其临床价值。方法对2002年1月至2009年6月在盐城市盐都区中西医结合医院进行分化型甲状腺癌治疗的124例患者的临床资料进行回顾性分析,根据患者癌肿的病变范围,主要采取患侧腺叶、峡部切除及对侧腺叶次全切除术进行治疗。结果术后出现并发症5例,占4.03%,其中术后出血2例,喉返神经损伤3例;随访84例,随访1～4(2.6±0.5)年,其中发生远处转移而死亡3例。结论手术仍然是治疗分化型甲状腺癌的主要治疗方式,根据患者的病理类型、肿瘤大小以及是否有颈部淋巴结转移等不同,采用不同的手术方式进行治疗。

Observation on the Efficacy of Different Surgical Operations for Differentiated Thyroid Carcinoma

Objective To observe the efficacy of different surgical operations for differentiated thyroid carcinoma and its clinical value.Methods The clinical data of 124 patients which received therapy of differentiated thyroid carcinoma from January 2002 to June 2009 in Yandu District Hospital of Integration of Traditional Chinese Medicine with Western Medicine were analyzed retrospectively.According to the extent of disease in patients with carcinoma,ipsilateral lobe and isthmus resection,subtotal resection of the contra lateral lobe were mainly resorted.Results 5 cases presented postoperative complications after surgery,which accounted for 4.03%,including 2 cases of postoperative bleeding,3 cases of recurrent laryngeal nerve injury.84 cases were followed up for 1 to 4(2.6±0.5) years,3 patients died for distant metastases.Conclusion Surgical means remains the primary treatment of differentiated thyroid carcinoma.Different surgical approaches should be adopted depending on pathological type of patients,tumor size and whether cervical lymph node metastasis.
