胆总管囊肿手术治疗体会

目的：探讨胆总管囊肿的诊断及手术治疗经验。方法：对我院1995～2002年间收治的9例原发性胆总管囊肿病人临床资料进行回顾性分析。结果：9例均行内引流，其中6例行囊肿切除、肝总管空肠Roux-en-Y吻合术，2例行胆总管囊肿与空肠Roux-en-Y吻合术，l例行囊肿大部分切除、囊肿空肠Roux-en-Y吻合术。术后随访6～24月，其中7例效果良好，l例术后1年因囊肿癌变死亡。结论：胆总管囊肿大部分临床表现不典型，术中仔细探查及胆道造影十分重要，术式以囊肿切除、肝总管空肠Roux-en-Y吻合术为佳，该术式操作简便、安全、术后效果好、并发症少。

Experience of Operative Treatment for Congenital Choledochal cyst

To Summarize the diagnosis and treatment of the congenital choledochal cyst (CCC ) .Methods; 9 cases ofcongenital choledochal cyst from 1995 to 2002 were retrospectively analyzed. Results; Internal drainage of congenital choledochal cysts were performed in 9 cases, includeing 6 cases of cysts excision with Roux - en - Y hepaticojejunostomy, 2 cases of choledoccho - cysto - jejunostomy Roax - en - Y, 1 cases of partial excision with choledoccho - cysto - jejunostomy Roux - en - Y. After 6-24 months follow - up, 7 cases were alive and healthy. 1 case died of cyst canceration 1 year after operation. Conclusions: The clinical symptoms of CCC are non - specific. Introoperative exploration and cholangilgraph is very important for its diagnosis. Excision of choledochal cyst and Roux - en - Y hepaticojejunostomy is the best method for treatment of CCC. Its main characteristics arc:Simple performance,safe,effective, less complicaton.
