152例IgA肾病中医体质类型与临床特征研究

目的:探讨IgA肾病患者的中医体质类型及其临床特征。方法:将152例IgA肾病进行体质分类,并记录其临床分型及检测相关指标。结果:IgA肾病中医体质类型分布差异较大,从高到低依次为气虚质(31.58%)、阴虚质(26.32%)、湿热质(19.08%)、血瘀质(7.9%)、阳虚质(5.9%)、气郁质(5.3%)、痰湿质(2.0%)、平和质(1.3%)及特禀质(0.6%);气虚质、阴虚质、湿热质、血瘀质、气郁质以无症状尿检异常型和镜下血尿型为主,阳虚质以大量蛋白尿型为主,痰湿质以高血压型为主,血瘀质可表现为血管炎型;气虚质、阴虚质、阳虚质各项指标呈逐渐加重的趋势,痰湿质体质指数和平均动脉压最高,湿热质表现为中等量蛋白尿,血瘀质和气郁质各项指标均处于"中间型"。结论:IgA肾病中医体质以气虚质、阴虚质和湿热质多见,体质类型与发病形式及客观指标密切相关。

Research of TCM constitution type and their clinical features of 152 patients with IgAN

Objective:To investigate the TCM constitution type and their clinical features.Methods:152 cases of IgAN were classified according to the constitution,recorded the clinical type and detected the relevant indicators.Results:The constitution type of TCM of IgAN distributed greater differently,In the order of Qi-deficiency(31.58%),Yin-deficiency(26.32%),Damp-heat(19.08%),Blood stasis mass(7.9%),Yang-deficiency(5.9%),Qi-quality(5.3%),Phlegm(2.0%),Gentleness(1.3%) and Special intrinsic quality(0.6%).Qi-deficiency,Yin-deficiency,Damp-heat,Blood stasis mass and Qi-quality mainly expressed as asymptomatic urine abnormality type and microscopic hematuria type.Yang-deficiency expressed as massive proteinuria type,Phlegm as hypertensive type and the blood stasis mass could show as vacuities type.The indicators of Qi-deficiency,Yin-deficiency and Yang-deficiency showed a gradual worsening trend,Phlegm up to the body mass index and mean arterial blood pressure,Damp-heat showed a moderate amount of proteinuria,Blood stasis mass and Qi-quality in the "intermediate" type.Conclusion:The constitution of TCM of IgAN mainly show as Qi-deficiency,Yin-deficiency and Damp-heat,and the type of constitution is closely related to the onset of the disease and objective indicators.
