急性结石性胆囊炎肝功能损害90例临床分析

目的探讨急性结石性胆囊炎症程度与肝功能损害的关系。方法将90例患者按炎症程度分为轻、中、重3型,设50例患者(无肝胆疾病)为对照组,针对4项肝功能指标:总胆红素(TBIL)、谷草转氨酶(AST)、谷丙转氨酶(ALT)和血浆白蛋白(ALB)与对照组两两比较统计学处理,应用t检验。结果轻型胆囊炎ALT、TBIL、AST、ALB与对照组比较均差异无统计学意义(P>0.05),而中、重型胆囊炎ALT、TBIL、AST均较对照组明显升高,ALB均较对照组明显降低,差异有统计学意义(P<0.05或P<0.01)。结论急性结石性胆囊炎炎症程度与肝功能损害呈正相关,保肝、消炎、尽早手术切除胆囊是肝功能恢复的较好方法。术前可通过肝功能损害程度作为判断炎症程度参考因素之一,以判断病情,预测后果,评估手术方案及手术时机。

Clinical analysis of liver function damage of 90 cases acute calculous cholecystitis

Objective To explore the relation between the time and the degree of inflammation in acute calculous cholecystitis and liver function damage.MethodsAccording to degree of inflammation of acute calculous cholecystitis,90 cases of acute calculous cholecystilis were divided into light,median and serious degree 3 groups.50 cases of no liver and bile duct diseases were set as control group.Four liver function indexes: TBIL,AST,ALT and ALB were determined and compared.ResultsTBIL,AST,ALT and ALB showed no obvious difference in the light inflammation of the gallbladder group than control group(P>0.05),However,ALT,TBIL、AST was remarkably higher in the groups of median and serious inflammation of the gallbladder than control group,ALB was remarkably decreased in the groups of median and serious inflammation of the gallbladder than control group,there were remarkable and most remarkable difference in the groups of median and serious inflammation of the gallbladder than control group(P<0.05 or<0.01).ConclusionThere is a positive relationship betwen the inflammation degree of acute calculous cholecystitis and the damage of liver function,It is suggested that the better way for the recovery of liver function is liver protection,cantrol of inflammation and performing cholecystectomy in time.
