CT导引经皮肺穿刺在肺部肿块诊断中的应用价值

目的探讨通过CT导引穿刺活检结果对肺部肿块的X线-CT影像诊断的价值及再认识。方法选取资料完整的肺部肿块CT穿刺活检／抽吸引流18例进行回顾分析，分析假阴性结果及影像表现。结果本组穿刺全部成功。其中出现少量气胸3例，少量肺出血2例，假阴性3例。结论薄层扫描技术有利于提高阳性率，活检结果关系到治疗方案的选择；肺结核基础上发生肿块应警惕肺癌的发生；巨块型肺癌坏死液化时其周壁可薄而均匀、非凹凸不平或厚壁空洞。

An evaluation of CT- guided lung biopsy for the diagnosis of pulmonary masses

Objective To improve understanding of X-ray and CT findings of lung mass through a retrospective study of CT - guided lung biopsy. Methods The complete clinical data of 18 cases of lung masses, on which CT - guided biopsy or aspiration were performed, were retrospectively analyzed for the false - negative reasons and imaging findings. Results Successful procedures were performed in all cases with complication of mild pneumothorax in 3cases and mild lung bleeding in 2 cases. And false - negative result was found in 3 cases. Conclusion Thin - slide CT scan helps to increase the positive rate. Biopsy results are closely related to clinical management. It should be concerned that lung cancer may occur when mass developing at the original lesions of tuberculosis, and large lung cancer may result in cavitation with thin and smooth wall instead of a rugged thick-walled cavitation.
