肾切除术前的肾动脉栓塞的疗效体会(附5例报告)

5例肾肿瘤患者行肾切除术前的肾动脉栓塞术 ,其中 4例在术中出血明显减少 ,与正常组织界线清 ,便于肿瘤切除。认为术前 3～ 7d行栓塞治疗术较好 ,无水乙醇比明胶海绵中断血运更好 ,无水酒精只要栓塞至二级分支足以中断血流。造影应显示供血血管、引流静脉、肿瘤血供 ,为肾栓塞和切除术作参考。

Towards Renal Artery Embolization Prior to Nephrectomy

Renal arterial embolization was performed in 5 cases of renal carcinoma prior to nephrectomy. As a result, in 4 of the 5 patients bleeding was markedly reduced during nephrectomy, prviding a clear borderline between the diseased tissues and the normal tissues for the removal of a tumor. Embolization was preferably performed 3 to 7 days prior to nephrectomy. To stop the renal blood supply, absolute ethanal proved to be more effective for the embolization than gelfoam, for absolute ethanal was effective enough to block the blood flow completely by occluding the secondary branches of the renal arteries alone. Angiography should be used to show up fine details in the renal arteries, drainage veins and tumor vessels for the reference of the embolization and nephrectomy.
