中晚期肝癌介入术中应用明胶海绵时机的探讨

摘    要:目的探讨肝癌肝动脉栓塞应用明胶海绵的时机。方法回顾120例肝癌患者在介入治疗中肝肿瘤的情况,就首次介入术中是否应用明胶海绵栓塞作一讨论和比较。结果在首次治疗操作过程中,对肿瘤较大、注入碘油抗癌药在肿瘤区域分布欠均匀时,仅以碘油抗癌药填充癌灶。待能够行第二次治疗时,用碘油抗癌药尽量充盈癌灶后,视病情加用明胶海绵颗粒栓塞的作法,可减少术后并发症,延长生存期。结论视病情而定掌握明胶海绵栓塞的时机,更有利于对肝癌肿瘤的控制,有效提高患者生存期。

Abstract:Objective To discuss on appropriate using time of gelatinum sponge during interventronaltreatment for advanced hepatocellular carcinoma. Methods 120 cases with hepatocellular carcinoma weretreated with interventronal treatment. Through the evaluation of the change of tumour survival, we havediscussed whethere transarterial embolization with gelatinum sponge should be used during the firstinterventronal treatment.Results During the first interventronal operation, only iodine oil antican-cer drug is used to fill the focus. When the second interventronal treatment is used and the focus isfilled with iodine oil, the usage of gelatinum sponge embolization is effective in alleviation post-operation complication and prolongation of survival time. Conclusion To determine the appropriateusage time of gelatinum sponge embolization according to the phase of disease is more effective in thecontrol of hepatocellular carcinoma and improve the survival of patients.
