经皮双头加压螺钉治疗股骨颈骨折并发症的原因分析

摘    要:目的 探讨经皮双头加压螺钉治疗股骨颈骨折并发症的原因及其相关对策。方法 总结 1995年 4月-2 0 0 2年 12月 46例经皮双头加压螺钉治疗股骨颈骨折的经验。对其中失败的 15例进行分类 ,探讨相关原因。结果  46例中有 15例出现并发症 ,并发症的类型分为 5型 :股骨头坏死、螺钉穿出股骨头、髋外翻、髋内翻、螺钉穿出股骨颈后方。患者的年龄、骨折的类型、骨的质量、骨折的时间、手术技术、手术适应症的选择是失败的相关原因。结论 选择经皮双头加压螺钉治疗股骨颈骨折应严格选择病例。对高龄不稳定型的股骨颈骨折治疗经皮双头加压螺钉不应首选。

Abstract:Objective TO analyse complications factors and relative strategies treating femoral neck with percutaneous hollow double heads compression screw.Methods Fourty-six cases of femoral neck fracture were treated by percutaneous hollow double heads compression strict in choices cases.Percutaneous hollow double heads compression screw from april 1995 to december 2002,15 cases were complications.The complications cases were analysed to find out the factors.Results Complications factors were divided into 5 types,Femoral head necrosise,Femoral head perforation of crew,Coxa valgas,Coxa varas,Screw cutting femoral neck,Failure relative factors were gegarded to age,Type of fracture,Quality of bone,Time of fracture,Opreation technique,and choice of operative indication.Conclusions Using percutaneous hollow double heads compression screw to treat femoral neck fracture must be screw should not be the first choice for treatment of old patient with unstable femoral neck fracture.
