曲安奈德益康唑乳膏致大疱性多形红斑1例

1例82岁男性患者,因双下肢红斑、水疱、糜烂性瘙痒3天,加重1天门诊就诊后直接收治入院。既往诊断为矽肺II期,否认药物、食物过敏。入院前因湿疹自行购买曲安奈德益康唑乳膏外用,用药次日出现红斑、瘙痒表现未停药,持续用药3天后皮肤损害加重。入院后确诊为大泡型多形红斑药疹,经抗炎、抗免疫、抗过敏、保肝护胃等治疗后好转出院。该案例的出现为日后使用曲安奈德益康唑类乳膏的广大湿疹患者敲响了警钟,使用该药后若出现不适需立即停药,并及时就医。

One case of Severe bullous erythema multiforme-type drug eruption suspected to be caused by triamcinolone acetonide iconazole cream

An 82-year-old male patient was admitted to the hospital directly due to erythema, blisters and erosive pruritus of both lower limbs for 3 days, aggravation for 1 day after outpatient treatment. He was previously diagnosed as stage II silicosis, and drug and food allergies were denied. Before admission, triamcinolone triamcinolone economazole cream was purchased for external use due to eczema, and the symptoms of erythema and pruritus appeared the next day. Skin damage was aggravated after 3 days of continuous use. The patient was diagnosed with bullous erythema multiforme drug eruption, which improved and discharged after anti-inflammatory, anti immune, anti allergic, liver and stomach protection treatment. The occurrence of this case has sounded the alarm for the majority of eczema patients who use triamcinolone acetonide econazole cream in the future. If they feel unwell after using the drug, they need to stop the drug immediately and seek medical treatment in time.
