坐浴外洗联合环状混合痔术治疗术后并发症随机平行对照研究

目的]观察坐浴外洗联合环状混合痔术治疗术后并发症疗效。方法]使用随机平行对照方法,将70例住院患者按抛硬币法随机分为两组。对照组35例环状混合痔术,术前患者清淡、半流质饮食。术前1d清洁灌肠。手术取右侧卧位,碘伏消毒,局部麻醉。肛镜下对直肠上动脉区及痔体注入30mL消痔灵液。外痔较大混合痔行外剥内扎术,外痔较小混合痔则行弧形切除缝合术。术后,将1枚消炎痛栓纳入肛门,并用凡士林纱布填压创面,最后压迫包扎固定。每日便后,温水坐浴外洗,结束后油纱条换药。治疗组35例坐浴外洗(黄柏30g,黄连15g,苦参、地榆、延胡索各30g,白芷15g),水煎500mL,排便后坐浴,时间30min。结束后生肌玉红膏纱条换药;环状混合痔术同对照组。连续治疗14d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。结果]治疗组痊愈33例,好转2例,无效0例,痊愈率94.29%。对照组痊愈27例,好转8例,无效0例,痊愈率77.14%。治疗组疗效优于对照组(P0.05)。结论]坐浴外洗联合环状混合痔术治疗术后并发症效果显著,值得推广。

Results ] The treatment group （ 33 cases cured, improvement in 2 cases, 0 no effect, the effective rate was 94.29%. And 8 cases of the control group, 27 patients were improved 0 cases, the effective rate was 77.14%. The treatment group curative effect is better than that of control group （ P〈0.05 ） .
