针灸联合康复训练治疗老年脑卒中偏瘫随机平行对照研究

[目的]观察针灸联合康复训练治疗老年脑卒中偏瘫疗效。[方法]使用随机平行对照方法,将130例门诊及住院患者按随机数字表法分为两组。对照组65例康复训练:强化护理服务,保证患肢位于正确位置,选床上抗痉挛体位,促使其患肢位于功能位,并强化抑制患肢痉挛模式锻炼,鼓励床上健肢的被动或主动锻炼,双手交叉上举运动,下肢进行桥式运动,骨盆平衡,直至患肢自主运动。强化日常生活能力,配合中医康复按摩,根据患者病情引导相应主动运动及速度运动,与此同时抑制患肢痉挛模式锻炼。治疗组65例取患侧肢体穴位:足三里、太冲穴、阳陵泉、解溪、申脉、太溪、承山、照海。每天选4个穴位,消毒,选提插捻转手法毫针深刺穴位操作,留针15~20min,1次/d,6次/周。康复训练同对照组。连续治疗7d为1疗程。观测临床症状、功能评分、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈11例,显效32例,有效13例,无效9例,总有效率86.15%。对照组痊愈5例,显效19例,有效17例,无效24例,总有效率63.08%。治疗组疗效优于对照组(P0.01)。功能评分两组均有改善(P0.01),治疗组改善优于对照组(P0.01)。[结论]针灸联合康复训练治疗老年脑卒中偏瘫效果显著,值得推广。

The treatment group 65 cases take side body points： foot three mile, lr 3 hole, gb 34, st 41, shen mai, too creek, bl 57, according to the sea. Every day to choose four meridians, disinfection, choose to insert turning style filiform needle deep acupuncture point operation, retaining needle 15 - 20 min, 1 / d, 6 times per week. Rehabilitation training with the control group. Observation of clinical symptoms and functional score, adverse reactions. Treatment as a course （ 7 d ）, to determine efficacy. [ Results ] Treatment group recovered 11 cases, 32 cases were markedly effective, effective in 13, 9 no effect, the total effective rate was 86.15%. The control group recovered in 5 cases, 19 cases were markedly effective, 17 cases effective, invalid 24 cases, total effective rate was 63.08%. The treatment group curative effect is better than that of control group （ P〈0.01 ） . Function score two groups have improved （ P〈0.01 ）, the treatment group improved better than that of control group （ P〈0.01 ） . [ Conclusion ] Acupuncture combined treatment of senile cerebral apoplexy hemiplegia rehabilitation training effect is remarkable, is worth promoting.
