清热凉血益气滋阴方联合西药治疗小儿特发性血小板减少性紫癜随机平行对照研究

目的]观察清热凉血益气滋阴方联合西药治疗小儿特发性血小板减少性紫癜疗效。方法]使用随机平行对照方法,将50例住院患者按随机数字表法随机分为两组。对照组25例口服强的松,2~3mg·kg-1·d-1,每日晨服,到血小板恢复,接近于正常后,逐渐减量,剂量可降到最低;血小板20×109/L患者,静滴血小板悬液。治疗组25例清热凉血益气滋阴方(生地15g,水牛角先煎20g,鸡血藤30g,当归7~8g,大枣10~15g,仙鹤草15~25g;血热妄行加茜草10g,大黄6g,白茅根15g;阴虚内热加女贞子10g,玄参10g;气不摄血加党参60g,黄芪15g,柴胡、升麻各6g);1剂/d,水煎200mL,早晚口服;西药治疗同对照组。连续治疗5d为1疗程。观测临床症状、血小板、不良反应。连续治疗2疗程,判定疗效。结果]治疗组显效5例,有效19例,无效1例,总有效率96.00%。对照组显效3例,有效17例,无效5例,总有效率80.00%。治疗组疗效优于对照组(P0.05)。结论]清热凉血益气滋阴方联合西药治疗小儿特发性血小板减少性紫癜效果显著,值得推广。

Objective]To observe the heat cool blood yiqi Yin fang combined western medicine curative effect in the treatment of children with idiopathic thrombocytopenic purpura.Methods]Using random parallel control method,50 patients hospitalized patients were randomly divided into two groups according to random number table method. Treatment group（25 cases of clear heat cool blood yiqi Yin fang（Shengdi 15g,Shuiniujiao 20g,Jixueteng 30g,Danggui 7 - 8g,Dazao 10 - 15g,Xianhecao 15 - 25g. Blood heat：add Qiancao10g,Dahuang 6g,Baimaogen 15g,Yin deficiency with internal and Nvzhenzi 10g,Xuanshen 10g;gas no ejection and Dangshen 60g,Huangqi 15g,Chaihu, Shengma each 6g）;1/d,boiling water 200 ml,oral sooner or later. Western medicine treatment with the control group. Continuous treatment 5 d to 1 course of treatment. Observation of clinical symptoms and adverse reactions. Continuous therapy treatment 2（d）10,judge curative effect. Result]The treatment group 5 cases had marked effect,effectively and 19 cases,1 had no effect,total effective rate was 96.00%. Control group 3 cases had marked effect,17 cases effective,5 no effect,the total effective rate was 80.00%. The treatment group curative effect is better than that of control group（P〈0.05）. Heat cool blood. Conclusion] yiqi Yin square combined western medicine treatment of children with idiopathic thrombocytopenic purpura effect is remarkable,is worth promoting.
