阑尾化瘀汤加清热消肿散外敷联合抗感染治疗化脓性阑尾炎术后并发腹腔脓肿随机平行对照研究

[目的]观察阑尾化瘀汤加清热消肿散外敷联合抗感染治疗化脓性阑尾炎术后并发腹腔脓肿疗效。[方法]使用随机平行对照方法,将70例住院患者按随机数字表法分为两组。对照组35例常规抗感染、输液等治疗及护理。治疗组35例阑尾化瘀汤(大黄后下15g,金银花、连翘各30g,牡丹皮、桃仁各9g,延胡索12g,川楝子15g,木香10g,甘草9g),1剂/d,水煎250mL,早晚口服。清热消肿散(芒硝100g,大黄、大蒜各50g),研磨成末并用醋调成糊状,敷于患处,1次/d。抗感染治疗同对照组。连续治疗14d为1疗程。观测临床症状、临床症状改善时间、卫生经济学指标、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈20例,有效12例,无效3例,总有效率91.43%。对照组痊愈16例,有效9例,无效10例,总有效率71.43%。治疗组疗效优于对照组(P0.05)。临床症状改善时间及卫生经济学指标(住院时间、住院费用)治疗组少于对照组(P0.05)。[结论]阑尾化瘀汤加清热消肿散外敷联合抗感染治疗化脓性阑尾炎术后并发腹腔脓肿效果显著,值得推广。

Appendix Stasis Tonga Heat Swelling Powder Combined with Anti Infection Treatment Suppurative Appendicitis Postoperative Intra-abdominal Abscess in a Randomized Controlled Study

[Objective] To observe the appendix stasis Tonga heat swelling powder combined with anti infection treatment suppurative appendicitis postoperative intra-abdominal abscess. [Methods] Using randomized controlled Methods,will be divided into two groups according to the random number table, 70 cases of hospitalized patients. 35 cases of the control group with conventional anti infection treatment and nursing,infusion etc.. The treatment group of 35 cases of appendiceal Huayu Decoction（Dahuang after 15g,Jinyinhua,Lianqiao each 30g,mudanpi,Taoren each 9g,Yanhusuo 12g,Chuanlianzi 15g, muxiang 10g,Gancao 9g）,1 doses of /d,water simmer in water 250mL,sooner or later oral. Clearing heat and dispelling（mangxiao 100g,Dahuang,Dasuan each 50g）,grinding into powder and vinegar to tune into a paste,apply to affected area,1 times /d. Anti infection treatment with the control group. Continuous treatment with 14d for 1 courses. The clinical symptoms,improve clinical symptom observation time,index,health economics,adverse reaction. 1 courses of treatment,the curative effect. [Results] The treatment group cured 20 cases,effective 12 cases,invalid 3 cases,the total efficiency of 91.43%. The control group cured 16 cases,effective 9 cases,invalid 10 cases,the total efficiency of 71.43%. The treatment group was better than control group（P〈0. 05）. The improvement of clinical symptoms and health economics indexes time（duration of hospitalization,cost of hospitalization）treatment group was less than that of the control group（P〈0. 05）. [Conclusion] The effect of intra-abdominal abscess appendix stasis Tonga heat swelling powder combined with anti infection treatment suppurative appendicitis after significant,worthy of promotion.
