彩色多普勒能量图在诊断盆腔实性肿物中的应用

目的 探讨CDPI对盆腔良恶性实质性肿物血流特点的诊断价值。方法 应用CDPI对67例盆腔肿物血流进行检测及对比分析。结果 卵巢良性实质性肿物内血流特点为周边见环状及星点状血流信号。卵巢恶性肿瘤内血流信号表现不一,多以内部血流为主,呈树枝状或网状分布,血流频谱为低阻力型。子宫肌层肌瘤以瘤体周围短棒状及条状血流信号为特征。浆膜下肌瘤以蒂部血流信号丰富。子宫内膜癌肿瘤内血流呈弥散分布伴低阻力型血流。绒毛膜癌内部呈“火球样”血流并伴低阻力高流速为特征。结论 CDPI能清晰显示肿瘤内部的血管分布,在盆腔良恶性肿物的血流形态学分布的显示各有其特征,是鉴别盆腔良恶生肿物的一种有价值的方法。

The Application of Color Doppler Power Imaging in Diagnosing Parenchyma Tumor in Pelvic Cavity

Objective To discuss the diagnostic value applying color Doppler Power Imaging (CDPI) and observe the blood flow characteristics of the parenchyma tumor in pelvic cavity. Methods Applying CDPI and color Doppler flow imaging(CDFI) to examine and analyze the blood flow characteristics of the tumor in pelvic cavity in 67 patient. Results The blood flow characteristics of the benign parenchyma tumor in ovary was the asterism shape;The blood flow sign in the malignant tumor in ovary was asymmetry and more of its distributing was diffuse, moreover its blood flow frequency spectrum was the low resistance type; The blood flow characteristics of the hysteromyoma in uterus muscle layer was the straight thread shape around the tumor's body; The blood flow sign of hysteromyoma in subserous layer was abundant in tumor pedicle. The blood flow mensurating value in carcinoma of endometrium was of diffusion and low resistance; The blood flow characteristic of choricarcinoma was of low resistance and high velocity, looking like "fireball shape". Conclusion There are respective characteristics in the blood flow distribution in the benign or malignant tumor in pelvity cavity, so it is of great significance for CDPI in discriminating the benign or malignant tumor in pelvic cavity.
