摘    要:目的:探讨现行管理体制下综合医院病房护士直接护理时间、间接护理时间的分配比例及不同作业特点科室之间的差异,为人力资源科学管理提供依据。方法:选择安徽省某省级三级甲等医院37个临床科室的护理人员为观察对象,对67项直接护理项目与17项间接护理项目进行频数测定,对16项间接护理项目按作业时间量以分钟为单位记录。结果:直接护理时间占护理总时间的61.80%。内科组与外科组间直接护理时间无统计学差异,但均与监护病房组有显著性差异。间接护理时间3组间差异无统计学意义。13项操作占100项护理操作时间的59.07%。适宜的床护比至少应1:0.5,该院护士仍处于缺编状态,缺编率为17.36%。结论:护理工作量测算作为医院护理工作状态指标,为优化护理人力资源管理提供依据。

Abstract:Objectives: To calculate the current direct nursing time and indirect nursing time in various wards of a general hospital; to explore the difference among different wards; and to offer evidence to scientific human resource management. Methods: Thirty-seven nurses from different clinical wards were observed. The frequencies of 67 direct nursing items and 17 indirect nursing items were measured and recorded. Among all the items, 16 indirect nursing items were measured in minute. Results: In the total nursing time, 61.80% of it was direct nursing time. There was no significant difference between the internal medical and surgery departments, but both of them were significantly different from the intensive care units. In 100 nursing items, 59.07% of them were occupied by only 13 items. The shortage rate of nurses in this hospital was 17.36%. Conclusions: The measurement of nursing work-load can be used as an index of nursing work-status of a hospital and as the evidence for optimal nursing human resource management.
