三维成像DSA在脑血管病诊断和治疗中应用价值的研究

摘    要:目的 探讨三维数字成像在脑血管疾病诊断和治疗中的应用价值.方法 应用GE Innova 3100平板DSA数字减影机.35例颅内动脉瘤患者,18例动静脉畸形患者,9例颈内动脉海绵窦漏.62例脑血管病病例行三维和二维DSA成像检查,应用二维数字减影血管造影患者的资料和应用三维数字减影血管造影患者的资料,对比分析三维和二维DSA曝光次数、造影剂的用量、患者接受的X光照射量和两种DSA图像特点.结果 应用三维数字血管造影(3D-DSA)可以减少总曝光次数,不仅在选取参考图像时曝光次数减少,而且在手术过程中曝光次数也减少.两组造影剂的用量无统计学差异.3D-DSA图像因可以从三维角度观察而能得到更全面的信息.结论 三维数字血管造影可以从多方位观察颅内血管的结构,可以提供更清晰的参考图像角度,从而可减少DSA的曝光数量,三维数字血管造影优于二维数字减影血管造影.

Abstract:Objective To study the value of 3D-DSA imaging to diagnosis and therapy of crerebrovascular disease (CVD). Methods Sixty-two patients with CVD, of whom, 35 suffered from intracranial aneurysms, 18 from arteriove-nous malformations and 9 from carotid-cavernous fistulas, were examined by GE Innova 3100 flat panel plate DSA machine. The data of 3D-DSA imaging including exposure time, dosage of contrast medium, X-ray irradiation dose and image features were compared with those of 2D-DSA imaging. Results 3D-DSA could reduce the total exposure time during the selection of reference pictures and the operation. There was insignificant difference in the dosage of contrast medium between 3D-DSA imaging and 2D-DSA imaging. The more information was observed by 3D-DSA compared to 2D-DSA. Conclusion The 3D-DSA can observe the multi-dimensional images of the intracranial vessels, provide clearer reference image and reduce the exposure dosage. Therefore, 3D-DSA is better than 2D-DSA.
