无创呼吸机辅助治疗急性左心衰效果观察

 目的 评价双水平正压无创通气(BiPAP)救治急性左心衰的临床疗效.方法 对42例急性左心衰患者,在常规药物抗心衰治疗基础上联用BiPAP呼吸机面罩正压通气治疗,测定动脉血pH、PaO2、PaCO2,观察血压、心率、呼吸及临床症状,并进行相关分析.同时与42例常规药物治疗组相对照.结果 治疗组呼吸、心率、血压均明显低于对照组(P＜0.05),SpO2明显高于对照组(P＜0.05);pH、PaO2、PaCO2和BE均较对照组明显改善(P＜0.05);治疗显效率为47.62%,明显高于对照组(P<0.05);且总有效率为92.86%,明显高于对照组的78.57%(P<0.05).结论 急性左心衰竭患者同时应用呼吸机结合常规药物治疗较之单纯药物治疗效果显著,值得临床推广运用.

Treatment of acute left heart failure with conventional therapy and bi-level positive airway pressure

Objective To evaluate the therapeutic effects of bi-level positive airway pressure on acute left heart failure.Methods Forty-two patients with acute left heart failure received combined conventional therapy and bi-level positive airway pressure,and were compared with 42 patients with acute left heart failure receiving traditional treatment as controls.The changes in the power of hydrogen,partial pressure of oxygen in artery,pulse oxygen saturation,partial pressure of carbon dioxide in artery,buffuer excess,blood pressure,heart rate,and breathing frequency,and clinical symptom were compared between the two groups.Results Compared with control group,the blood pressure,heart rate,breathing frequency decreased significantly(P<0.05);pulse oxygen saturation increased significantly(P<0.05);the power of hydrogen,partial pressure of oxygen in artery,partial pressure of carbon dioxide in artery,and buffuer excess were improved more significantly(P<0.05);the marked response rate and the total effective rate increased significantly(P<0.05).Conclusions The combin conventional therapy and bi-level positive airway pressure has a good efficacy in the patients with acute left heart failure.
