鼻唇沟皮下蒂真皮瓣悬吊及表情肌紧缩行颊部面瘫修复

面瘫的整形修复方法很多。近年来，随着显微外科技术的应用，神经肌肉移植在面瘫修复中的应用受到广泛注意。但面瘫治疗仍是一难题，有待进一步努力。为此作者自１９９０年以来对１１例面瘫患者采用以皮下蒂鼻唇沟真皮瓣悬吊，同时紧缩该处表情肌的手术方法，取得成功。静态外形显著改观，动态外形亦有一定改善。手术方法简易，无需多处切口，技术亦易掌握。术后随访４个月至２年，真皮瓣悬吊未见松弛，１例发生皮脂腺囊肿，经手术切除不再复发，外形未受影响。此种新技术简单、可靠，易于推广。

Facial palsy reconstruction using both subcutaneous pedicle dermal flap of nasolabial fold and shortened muscles of facial expression

There are many ways for facial palsy reconstruction. In recent years,by using microsurgery technique,the free nerve transplantation, free muscular flap transplantation have been made for facial palsy reconstruction. But the results are not very perfect. So there are still some space to improve in this field.We developed the new surgical technique for facial palsy reconstruction. Since 1990, the subcutaneous pedicle dermal flap harvested from nasolabial fold has been used for static suspension paralyzed angle of the mouth on eleven facial palsy patients. At the same time the muscles of facial expression were also shortened for elevating the affected angle of the mouth. The results were satisfactory.Not only the effected side of mouth was roughly kept in normal position, but also the shortened muscles even could slightly movable.After four months to two years of postsurgery follow up,the dermal flap kept in normal position.One of these cases suffered from a sebaceous cyst in the incision ,but the figure was good after excision,and since then he never suffered from it again.This technique is easy to spread because it is simple and reliable.
