胎儿窘迫母血、脐血中AOPP水平与新生儿窒息的关系

摘    要:目的探讨胎儿窘迫母血、脐血中晚期氧化蛋白产物(AOPP)水平与新生儿窒息的关系,为临床预防和早期处理胎儿窘迫和新生儿窒息提供实验室依据。方法应用Witko—Sarsat法加以改进检测胎儿窘迫组和对照组母血、脐血中AOPP水平,观察各组新生儿窒息的发生情况。结果①胎儿窘迫组母血及脐血中AOPP水平分别为139.89±12.82μmol/L,139.52±17.28μmol/L,对照组分别为104.67±11.63μmol/L,109.56±11.67μmol/L,胎儿窘迫组显著对照组,差异有显著性(P〈0.05);②胎儿窘迫持续时间≥30min与持续时间〈30min相比较,脐血中AOPP有显著差异(158.38±8.03μmol/L vs131.43±13.36μmol/L,P〈0.05);母血在胎儿窘迫不同持续时间段差异无显著性;③胎儿窘迫组新生儿窒息发生11例,其中轻度窒息9例,重度窒息2例,与脐血中AOPP的变化密切相关。结论氧化应激损伤与胎儿窘迫及新生儿窒息关系密切,检测脐血AOPP水平有助于评估胎儿窘迫和新生儿窒息。母血中AOPP水平与胎儿缺氧的关系有待进一步研究。

Abstract:Objective To investigate whether the level of advanced oxidation protein products(AOPP) in maternal blood and cord blood have relations to fetal distress and neonatal asphyxia. Methods Maternal blood and cord blood were collected from 30 parturients who were delivered by emergent cesarean section for fetal distress. AOPP was also determined in 30 parturients who were delivered by elective cesarean section ( controls). Fetal distress and neonatal asphyxia were investigated in the two groups. Results ① The level of AOPP was higher in maternal blood in fetal distressed group than that in controls (139.89 ± 12.82 μmol/L vs 104.67 ± 11.63 μmol/L, P 〈 0.05 ). AOPP concentration was higher in cord blood in fetal distressed group than that in control group (139.52 ±17.28 μmol/L vs 109.56 ±11.67 μmol/L, P 〈0.05). ②AOPP level of cord blood in ≥ 30 min fetal distressed group was higher than that in 〈 30 min fetal distressed group (158.38 ± 8.03 μmol/L vs 131.43 ± 13. 36 μmal/L, P 〈0.05 ). ③There were 9 cases of moderate neonatal asphyxia and 2 cases of severe neonatal asphyxia in fetal distressed group and there was apparently relationship between the level of the disease and the concentration of AOPP. Conclusion The results indicate that the incidence of fetal distress and neonatal asphyxia is closely related to peroxidation. The level of AOPP in cord blood may evaluate the fetal, distress and neonatal asphyxia. The relation between the level of AOPP in maternal blood and fetal distress and neonatal asphyxia is still unclear.
