卡尼汀对失血性休克兔复苏过程中血乳酸和剩余碱变化的影响

摘    要:目的:观察卡尼汀对失血性休克兔复苏过程中血乳酸和剩余碱变化的影响,以评估其对失血性休克复苏的作用。方法:24只家兔随机分成4组:生理盐水组、生理盐水加卡尼汀组、高渗盐溶液组和高渗盐溶液加卡尼汀组,在休克前(T1)、休克末期(T2)、复苏后1h(T3)、2h(T4)、4h(T5)、6h(T6)时间点对家兔的平均动脉压、心率进行检测,抽取血液进行血乳酸(BL)、剩余碱(BE)的测定。结果:复苏后,各组BL值随时间呈现逐渐下降,生理盐水加卡尼汀组各时间点BL显著低于生理盐水组(P〈0.05),生理盐水加卡尼汀组在复苏后2小时BL下降最为明显(6.97±2.56)mmol/L,在4小时又有小幅度升高(7.84±1.84)mmol/L。高渗盐溶液加卡尼汀组在复苏后1小时及2小时BL值明显低于高渗盐组(P〈0.01),复苏后整体的BL值变化与高渗盐组没有显著差别。结论:在失血性休克复苏早期使用卡尼汀,对减少乳酸的生成、保护组织细胞,减少因无氧代谢所造成的损伤有一定的意义。
