微量法检测氟康唑、伊曲康唑和特比萘芬对致病性真菌的敏感性

摘    要:目的 根据美国国家临床试验标准化委员会(NCCLS)最新公布的M27-A方案,检测氟康唑(FCA)、伊曲康唑(ICZ)和特比萘芬(TBF)对致病性真菌的敏感性。方法 采用NCCLSM27-a方案微量法,检测53株酵母菌、31株曲霉和31株皮肤癣菌对三种常用抗真菌药物的敏感性。结果 氟康唑对大部分念珠菌的敏感性较好,对皮肤癣菌的敏感性较差,伊曲康唑对真菌具有广谱抗真菌作用,而特比萘芬对皮肤癣菌高度敏感,少数菌株对一种或一种以上药物达到耐药性判定标准,表明临床上耐药现象存在。结论 NCCLSM27-A方案微量法具有准确性强、重复性好的特点,也可用于丝状真菌的药敏实验,奉研究结果提示我国临床上亦存在耐药菌株感染现象,应引起重视。

Abstract:Objective To test the susceptibility of our country clinical isolates to three kinds of antifungal agents which were often used in the clinical practice. Methods The minimum inhibitory concentration(MIC) of pathogenic yeast and Der-matophytes were conducted according to National Committee for Clinical laboratory Standard (NCCLS)M27-A Method. These isolates included 53 Candidaspp; 31 Aspergillus and 31 Dermatophytes. Results Most of the 53 tested Candida spp were sensitive to the tested FCZ. Most of them were resistant to TBF. The pathogenic fungi were extentivelly sensitive to 1CZ. Dermatophytes was high sensitive to TBF. Conclusion NCCLS method was accuracy and reproducibility. Meanwhile M27-A standards could be applied to antifungal susceptibility testing of filamentous fungi such as Aspergillus and Dermatophytes. Therefore the microdilution method may have clinical utility in laboroatory in antifungal susceptibility testing of both yeasts and filamentous fungi. In the meantime, our results suggested that drug-resistant strains also existed in our country's clinical practice, so attention must be paid to such cases.
