NP方案化疗联合三维适形放疗治疗晚期非小细胞肺癌疗效观察

目的:探讨长春瑞滨加顺铂方案化疗联合三维适形放射治疗局部晚期非小细胞肺癌的毒性反应及疗效。方法:将42例局部晚期非小细胞肺癌患者,随机分为放化疗联合组和单纯化疗组各21例。放化疗联合组NP方案诱导化疗2个周期后即刻或休息1～2 d后行三维适形放疗,放疗结束后再行NP方案化疗4个周期;单纯化疗组仅予NP方案化疗6个周期。两组均在治疗结束后1个月评价毒副反应及疗效。结果:放化疗联合组近期疗效明显高于单纯化疗组,两组比较差异有统计学意义(P<0.05),毒副反应较单纯化疗组有所增加,但经对症治疗均能耐受,所有患者均能顺利完成治疗计划。结论:三维适形放疗联合NP方案治疗局部晚期非小细胞肺癌有较好的近期疗效,毒性反应经对症处理后可以耐受,远期疗效值得进一步研究。

Outcomes of NP regimen as chemotherapy combined with three-dimensional conformal radiotherapy in advanced non-small cell lung cancer

Objective：To evaluate the toxicity and efficacy of NP regimen vinorelbine（NVB） ＋cisplatin（DDP）]as chemotherapy combined with three-dimensional conformal radiotherapy （3D-CRT） for locally advanced non-small cell lung cancer （ NSCLC ） .Methods： Forty-two patients with locally advanced NSCLC were randomly allocated to radiation group and single chemotherapy group （n =21 for each group） .The concurrent chem-otherapy group received NP regimen as induction chemotherapy for 2 cy-cles and additional 3D-CRT immediately or at intervals of one to two days of the first-line regimen,and then were given adjuvant NP regimen for 4 cycles followed by the radiotherapy .The single chemotherapy group were exclusively given NP regimen for 6 cycles.The two groups were evaluated in one month after the therapy for the toxicity and efficacy .Results： The short-term therapeutic effect was significantly better in the combined ra-diochemotherapy group than that of the single chemotherapy group , and the two groups were statistically different（P〈0.05）.Although the toxici-ty appeared increased as compared to the single chemotherapy group ,yet all patients was well tolerated after symptomatic treatment and successfully gone through the therapy plan.Conclusion： 3D-CRT combined with NP regimen may lead to better short-term effects on the patients with locally advanced NSCLC,given that the toxic reaction was managed symptomati-cally.And its long-term effects remain yet to be determined.
