病毒性肝炎门静脉血流动力学与若干甲襞微循环指标的相关性研究

目的 :探讨慢性肝炎和肝炎后肝硬化患者肝门静脉血流参数与甲襞微循环可测量指标之间有无相关性。方法 :将观察病例分为慢性肝炎组和肝硬化组二组 ,同时设对照组 2 0例 :主要观察指标为肝门静脉主干内径 (Dpv)、血流速度 (Vpv)、血流量 (Qpv)、和脾静脉内径 (Dav)、血流速度 (Vsv)、血流量 (Qsv)以及输入枝管径、输出枝管径、袢顶径和微血流速度 ,将所得数据进行统计学处理。结果 :Dpc、Vpc、Qpv在三组之间相比均有极显著性差异 (P <0 .0 1 ) ;Dsv、Vsv、Qsv在正常对照组与肝硬化组之间相比 ,差异极显著 (P <0 .0 1 ) ;就输入枝和袢顶径而言 ,慢肝组与肝硬化组有极显著性差异 (P <0 .0 1 ) ;就微血流速度而言 ,三组之间有极显著性差异 (P <0 .0 1 )。结论 :肝硬化患者肝门静脉血流参数与甲襞微循环可测量指标之间有一定的相关性。

Related Study between Liver Portal Hemodynamics and Some Indexes of Nailfold Micro-circulation in Cases of Viral Hepatitis

To investigate related befween the liver portal hemod ynamics and some indexes of nailfold microcirculation in the cases of chromic ha patitis and cirrhosis of the liver(Lc).Method: The cases was divided into two groups which were the chr omic hapatitis group and Lc group 20 normal controls by using comparative analys is.Internal diameter,velocity,discharge of lever portal(Dpy,dpv,Qpv),and interna l diameter,velocity,discharge of spleen vein(Dsv,Vsv,Qsv) and the nailfold mio rociroulation of arteriole limb,venous limb,diameter of capillary top,velocity o f microfolw were observed.All the data,were deal with statistics.Results: To compare the experement results that Dpv,Vpv,Qpv were higher significantly than the other groups( P<0.01 ),also,Dsv,Vsv,Qsv higher than the groups of chronic Hepatitis and Lc( P<0.01 ).The Nailfold miorocircul ation of the diameter on alteriole limbs and the topes of capillaries were highe r significantly chronic hapatitis group and Lc group( P<0.01 ),also velocity o f micfroflow was significantly higher among three groups( P<0.01 ) too.Conclusion: Indead,there was relation ship between some indexes of liver,portal hemodynamics and the nailfold of microcirculation in patients wi th liver cirrhosis.
