火炬椎弓根内固定器械的研制及临床生物力学研究

摘    要:目的:后路短节段椎弓根内固定器械治疗胸腰椎骨折及腰椎滑脱安全且有效,介绍自行研制的火炬内固定器械及在胸腰椎骨折及腰椎滑脱中的生物力学测试。方法:通过器械生物力学测试,临床治疗14例患者(骨折10例、滑脱4例),对复位及固定效果进行观察。结果:术后胸腰椎骨折均满意复位,腰椎滑脱中3例退行性滑脱未完全复位,1例崩裂性滑脱复位满意,随访均无高度丢失、断钉、松动、椎体滑移加重等并发症。结论:该器械结构简单、操作简便,固定坚强、稳定,但对腰椎滑脱的治疗有一定局限性,其分离复位间距的适当量化对评价手术效果有一定的指导意义。

Abstract:AIM:It is safe and efficient to treat thoracolumbar fracture and lumbar spondylolithesis with posterior short segement pedicle internal instrument. This study aimed to introduce the preliminary clinical application of self designed torch pedicle internal instrument to thoracolumbar fracture and lumbar spondylolisthesis and to investigate its biomechanics. METHODS:After biomechanics test,14 patients received the treatment of Torch pedicle instrument, including 10 cases of fracture(6 males and 4 females with an average age of 38.4 years)and 4 cases of spondylolithesis(4 females with an average age of 46.8 years).The reduction and fixation results were observed. RESULTS:The reduction was satisfactory in thoracolumbar fractures and lumbar spondylolithesis, but malreduction was found in 3 cases of degenerative spondylolisthesis and 1 case of cracked spondylolithesis was with the complete reduction.There were no vertebrae height losing,pedicle screws breakage and loosening,vertebrae spondylolithesis increasing et al in the follow up.
Keywords:Torch instrument is not only simple in structure and procedure  but also immovable and stable in fixation  however  there is a limit to the treatment of lumbar spondylolithesis.And the proper quantitative disposal of its spacing between separation
