应用局部皮瓣 压缩-牵引成骨技术修复小腿创伤后胫骨及软组织缺损

摘    要:研究背景 创伤后严重骨缺损合并皮肤软组织缺损一直是外科治疗的难题。本研究旨在探讨应用局部皮瓣及Ilizarov成骨技术修复小腿创伤后胫骨及软组织缺损。方法 2006年3月-2008年2月收治16例患者,男14例,女2例,平均年龄31.5岁,创伤后至入住本院行第一次手术平均时间为14.4周,清创后平均软组织缺损和胫骨缺损分布为为92.9 cm2和8.7cm。所有患者均以局部皮瓣修复创面,二期应用压缩-牵引成骨技术修复胫骨缺损。结果 2个肌皮瓣、14个逆行岛状皮瓣用于修复创面,所有皮瓣均成活。15例患者愈合后双侧下肢等长,1例患者患肢差异1.5cm。1例应用Ilizarov环形外架患者针道感染、踝关节僵硬。所有患者均能自行行走。15例患者重返工作,1例失业。所有患者均按照Paley方法进行骨及功能评分,骨评分结果为14例优秀、2例良好;功能评分13例优秀、2例良好、1例较差。结论 结果提示,局部皮瓣 压缩-牵引成骨技术可用以修复小腿创伤后胫骨合并软组织缺损,局部皮瓣是一种安全、有效的修复小腿软组织缺损的方法。

Abstract:Background: Treatment of extensive post-traumatic composite bone and soft tissue defects remains a complicated therapeutic problem and a challenge for surgeons . Here we investigate the application of local flaps and Ilizarov osteogenesis in reestablishment of combined severe defects of tibial bone and soft tissue. Method: Sixteen patients with bone and soft tissue defects were included. The mean age of the patients was 31.5 years. The average time from injury to initial surgery was 14.4 weeks. The average soft tissue and bone defect sizes were 92.9 cm2 and 8.7 cm, respectively. Local flaps were created to reconstruct soft tissue defects. The Ilizarov external fixator (IEF) or the Orthofix Limb Reconstruction System (OLRS) was used to reconstruct bony defects using delayed distraction osteogenesis.
