透明质酸钠在抗青光眼术后浅前房的应用

摘    要:目的探讨透明质酸钠在治疗抗青光眼术后浅前房的临床疗效。方法对154例(163眼)各类青光眼行小梁切除术,有51例(58眼)发生浅前房,其中21例(23眼)前房注入空气,23例(26眼)注入透明质酸钠使前房形成,观察两组手术效果、手术成功率及术中术后并发症。平均随访16.5月。结果在21例(23眼)前房注气者中,一次手术成功率为82.61%,发生前房积血26.08%,炎症反应82.61%;在23例行前房内注入透明质酸钠者,其一次手术成功率为100.00%,发生前房积血3.85%,炎症反应15.39%,差异有统计学意义。(P<0.01)。结论对小梁切除术后前房为浅Ⅰ~Ⅱ级,经药物治疗5~7天效果不佳者,或前房为浅Ⅱ级以上者,应及早手术,前房注入透明质酸钠。可提高手术成功率。

Abstract:Objective To discuss the use of sodium hyaluronate in the treatment of shallow anterior chamber after filtering surgery of glaucoma.Methods 51 of 154 cases occured shallow anterior chamber after the trabeculectomy surgery.21 of 51 cases were treated by injecting air into anterior chambers.23 of 51 cases were treated by injecting NAHA into anterior chambers.Observed the effect and the rate of success and complications after operation. Results The rate of success after operation that injecting air was 82.61% and that injecting NAHA was(100.00%).There was significant difference between two groups((P<0.01)).The rate of hemorrhage and inflammation in injecting air group was 26.8% and 82.61%,that of the injecting NAHA group was 3.85%and 15.39%.There was significant difference between two groups((P<0.01)).Conclusion The use of sodium hyaluronate in shallow anterior chamber after the trabeculectomy surgery can decrease the complication and increase the successful rate.
