耳穴贴压对缓解阑尾炎术后伤口疼痛及腹胀的影响

目的探讨细节护理对耳穴贴压缓解阑尾炎术后伤口疼痛及腹胀的影响。方法 2011年1月—2013年9月间,在本院外科进行阑尾炎手术并使用耳穴贴压的患者中随机选取160例,随机分为A组与B组,每组80例。A组在常规护理的基础上给予以疼痛护理及心理护理为主的细节护理,B组仅给予常规护理。分别观察两组患者术后4h、8h、12h、24h、36h、72h伤口疼痛程度,术后排气时间,出院时对护理满意度及耳穴贴压的依从性。结果 A组患者各时间段疼痛评分均低于B组,其中术后12h至术后72h各时间段疼痛评分差异有统计学意义(P0.05);A组患者疼痛时间和程度明显低于B组,差异有统计学意义(P0.05);A组患者术后肛门排气平均时间显著小于B组,两组比较差异有统计学意义(P0.05);A组患者护理满意度(96.3%)明显高于B组患者的护理满意度(75.0%),两组比较差异有统计学意义(P0.05);且A组护理非常满意人数显著多于B组,不满意人数显著少于B组,差异有统计学意义(P0.05);两组患者总依从性比较无显著差异,但A组患者完全依从人数显著高于B组,差异有统计学意义(P0.05)。结论耳穴贴压对于缓解阑尾炎术后伤口疼痛及腹胀具有良好的临床效果,而以疼痛护理及心理护理为主的细节护理可以明显的提升患者对耳穴贴压的依从性及护理满意度,值得临床推广。

The Effect of Ear Acupoint Plaster on Alleviating Wound Pain and Distension after Appendectomy

Objective To investigate the effect of detail nursing care on wound pain and distension by auricular plaster therapy after appendectomy.Methods From January 2011 to September 2013, 160 cases were randomly selected from those who used auricular plaster therapy after appendectomy in our hospital , which were randomly divided into group A and group B , with 80 cases in each group .Group A was given details nursing ( including pain nursing and psychological nursing ) besides routine nursing .Group B was given routine nursing after operation.All those were observed in patients with 4h, 8h, 12h, 24h, 72h, 36h, including degree of pain, postoperative exhaust time, nursing satisfaction and compliance to auricular plaster therapy .Results Pain scores of group A were lower than those of the group B in each time, and the difference was statistically significant from 12h and 72h after operation (P<0.05).The pain time of the group A was significantly lower than that of the group B , and the difference was statistically significant ( P<0.05).The average anal exhaust time of group A was less than that of the group B , and the difference was statistically significant (P<0.05).The nursing satisfaction of the group A (96.3%) was better than that of group B (75%), and there were significant differences between them (P<0.05).Patients who were very satisfied with nursing in group A were significantly more than those of the group B , and the number who was not satisfied was significantly lower than that that of the group B , and there were statistically significant differences (P<0.05).The number of full compliance patients in group A were significantly higher than those of the group B , and the difference was statistically significant (P<0.05).Conclusion The auricular plaster therapy for alleviating appendicitis postoperative wound pain and abdominal distension has good clinical effect , and the detailed nursing , which mostly includes pain nursing and psychological nursing can obviously improve the main effect of auricular plaster therapy, which is worthy of clinical promotion .
