柴胡疏肝合归脾汤治疗脑梗死后抑郁症的研究

目的探讨柴胡疏肝散合归脾汤加减治疗脑梗死后抑郁症的疗效安全性。方法将108例脑梗死后抑郁症患者随机分为研究组54例,对照组54例。两组均在脑梗死常规治疗基础上分别给予中药汤剂合阿米替林和单独阿米替林治疗,并于治疗前及治疗后1周,2周,4周,6周末分别采用汉密顿抑郁量表(HAMD)评定临床疗效。结果治疗2周末研究组汉密顿抑郁量表评分较治疗前显著下降(P0.05),对照组4周末显著下降(P0.05),在不同时间点两组间的差异比较有显著性(P0.05)。研究组19%出现口干、便秘、乏力、嗜睡等不良反应。对照组有47%患者出现以上反应。结论柴胡疏肝散合归脾汤加减辅助治疗脑梗死后抑郁症起效快,安全性好,疗效持久,依从性好,且能有效改善阿米替林的常见不良作用。

Research on Chaihu Shugan Powder combined with Guipi Decoction in the Treatment of Post-celebral Infarction Depression

Objective To explore the efficacy and safety of the modified Chaihu Shugan powder combined with Guipi decoction in the treatment of post-celebral infarction depression .Methods 108 patients with post-celebral infarction depression were randomly divided into research group of 54 cases and control group of 54 cases.The control group was given amitriptyline and conventional drug treatment .The research group was given Chinese medicine decoction on the basis of amitriptyline and routine treatment .The clinical effect was evaluated by HAMD before and after treatment at the end of the 1w, 2w, 4w and 6w, respectively.Results The HAMD score of the research group decreased significantly at the end of 2 weeks during treatment ( P<0.05 ) , the HAMD score of the control group decreased significantly at the end of 4 weeks during treatment ( P <0.05 ) , and there was significant difference in the comparison between the two groups at all different points.In the research group, 19%patients had adverse reactions such as dry mouth , anorexia, fatigue, sleepiness and so on.In the control group , 47%patients had adverse reactions .Conclusion The modified Chaihu Shugan powder combined with Guipi decoction in the treatment of post-celebral infarction depression had fast curative effect , good safety , lasting effect and good compliance , and can effectively improve the common adverse effect of amitriptyline .
