左炔诺孕酮宫内缓释系统治疗子宫腺肌症中远期效果的研究

目的：研究左炔诺孕酮宫内缓释系统（LNG-IUS）治疗子宫腺肌症的中远期效果,以助于临床应用及临床咨询工作。方法通过比较33例子宫腺肌症患者LNG-IUS治疗前后的疼痛评分、月经量PBAC评分、血红蛋白水平变化,评估其中远期疗效。结果 LNG-IUS治疗前和治疗1、2、3、4、5年后的疼痛评分分别为（7.7±2.3）、（2.0±2.0）、（1.9±1.8）、（1.8±1.8）、（1.8±1.4）、（1.4±0.9）分,LNG-IUS治疗前和治疗1、2、3、4、5年后的疼痛评分比较,差异有统计学意义（P〈0.01）；LNG-IUS治疗前和治疗1、2、3、4、5年后的月经量PBAC评分分别为（126.82±78.51）、（7.58±6.31）、（7.46±5.43）、（7.35±4.06）、（7.28±3.28）、（6.98±2.19）分,LNG-IUS治疗前和治疗1、2、3、4、5年后的月经量PBAC评分比较,差异有统计学意义（P〈0.01）；LNG-IUS治疗前和治疗1、2、3、4、5年后的血红蛋白分别为（111.41±19.32）、（121.38±8.56）、（125.42±8.02）、（128.65±7.21）、（127.61±4.05）、（128.49±2.96）g/L,LNG-IUS治疗前和治疗1、2、3、4、5年后的血红蛋白水平比较,差异有统计学意义（P〈0.05）。结论 LNG-IUS治疗子宫腺肌症能有效缓解疼痛、减少月经量及提高血红蛋白水平。

The medium-term and long-term effect of levonorgestrel intrauterine sys-tem in the treatment of adenomyosis

Objective To evaluate the medium-term and long-term effect of levonorgestrel intrauterine system （LNG-IUS） in the treatment of adenomyosis to to contribute to the clinical application and clinical advisory work. Methods Eval-uating the the medium-term and long-term effect of LNG-IUS by comparing the change of pain score,menstrual flow PBAC score and level of serum hemoglobin of 33 women with adenomyosis before and after treatment. Results Pain score was （7.7±2.3）,（2.0±2.0）,（1.9±1.8）,（1.8±1.8）,（1.8±1.4）,（1.4±0.9） points respectively before and after 1,2,3,4,5 years treatment with LNG-IUS,menstrual flow PBAC score was （126.82±78.51）, （7.58±6.31）, （7.46±5.43）, （7.35±4.06）, （7.28±3.28）, （6.98±2.19） points respectively before and after 1,2,3,4,5 years treatment with LNG-IUS, hemoglobin was （111.41±19.32）,（121.38±8.56）,（125.42±8.02）,（128.65±7.21）,（127.61±4.05）,（128.49±2.96） g/L respectively before and af-ter 1,2,3,4,5 years treatment with LNG-IUS and there was a statistical difference of pain score,menstrual flow PBAC score and hemoglobin level before treatment compared with after 1,2,3,4,5 years treatment （P〈0.01 or P〈0.05）. Con-clusion LNG-IUS can effectively release pain,reduce menstrual flow and raise level of hemoglobin of women with ade-nomyosis.
