我院心外科I类切口手术围手术期抗菌药物的应用调查

目的：调查我院心外科I类切口手术围手术期抗菌药物的应用情况，为临床合理用药提供参考。方法：随机抽取我院2012年1-6月100例心脏手术I类切口围手术期使用抗菌药物的病历，并对数据进行统计分析。结果：100份病历中，抗菌药物初次给药时机选择不合理的8例，术后使用抗菌药物疗程不合理的16例，抗菌药物品种选择不合理的14例。联合用药共33例，细菌培养送检2例，抗菌药物的联合应用较好。抗菌药物初次给药时间在术前0.5~2 h的病历占76%，手术时长〉3 h或失血〉1500 mL的病历术中追加抗菌药物的比例为9%，用药时长〈72 h的病历占71%，用药品种选择基本合理。结论：我院心外科I类切口手术围手术期抗菌药物的使用基本合理，将进一步加强对抗菌药物的监督管理，促进其在临床的合理使用。

Investigation analysis of antibacterials used in type I incision of cardiac operations during perioperative period in our hospital

Objective：To explore the application of antibacterials in cardiac surgical procedures during perioperative period in our hospital, and provide the reference for rational drug use. Methods：A total of 100 cases undergoing open-heart surgery with typeⅠincision from January 2012 to June 2012 were randomly selected and statistically analyzed. Results：Among the 100 cases, there were 8 cases with irrational administration time, 16 cases with irrational duration in postoperative use of antibacterials and 14 cases with unsuitable categories of antibacterials drugs. There were 33 cases with drugs combination and 2 cases with bacterial culture. The rate of cases administrated antibacterials within 0.5-2 h before operation was 76%. The rate of appended antibacterials during operation in the cases with operation time more than 3 h or blood loss more than 1500 mL was 9%. The rate of the cases with administration time within 72 h was 71%. The combination of drug use and the selection of antibacterials categories were basically rational. Conclusion：The utilization of antibacterials in typeⅠincision of cardiac operations during perioperative period in our hospital was basically rational. The supervision and management of antibacterials must be strengthened to promote the rational use of drugs.
