摘    要:目的观察高血糖对大鼠脑出血后血肿周围脑组织ATP和乳酸含量的影响,探讨脑出血后高血糖对脑损伤的可能机制。方法采用自体血注入法建立脑出血模型,将80只SD大鼠随机分为:假手术组(5只)、单纯脑出血组(25只)、高血糖A组(2 g/kg 50%葡萄糖,25只)和高血糖B组(4 g/kg 50%葡萄糖,25只)。观察各组大鼠术前、术后1、6 h血糖变化;测定各组大鼠术后1、3、7、10和14天血肿周围脑组织ATP和乳酸含量变化。结果与假手术组比较,其他3组术后1、6 h血糖均明显升高,术后1、3、7、10和14天ATP含量均明显降低,乳酸含量明显升高(P<0.05)。与单纯脑出血组比较,高血糖A组和高血糖B组大鼠术后1、6 h血糖明显升高,术后1、3、7、10和14天ATP含量明显降低,乳酸含量明显升高(P<0.05)。结论脑出血后高血糖可加重脑组织能量代谢障碍,加重脑损伤,损伤的程度与血糖浓度有关。

Effect of hyperglycemia on energy metabolism disorder after cerebral hemorrhage in rats
Abstract:Objectives To observe the effect of hyperglycemia on ATP and lactic acid content in perihematoma brain tissue after intracerebral hemorrhage,and to explore the possible mechanism of effect of hyperglycemia on brain damage. Methods Autologous blood injection method was used to establish cerebral hemorrhage model. Eighty SD rats were randomly divided into sham operation group(5 rats), simple cerebral hemorrhage group(25 rats), hyperglycemia after cerebral hemorrhage group(further divided into injecting glucose 2 g/kg and 4 g/kg groups of 25 rats each). Changes in blood glucose in each group were observed before operaton and 1 and 6 h after operation. Changes of ATP and lactic acid contents in perihematoma brain tissue in each group were determined 1,3,7,10,14 days after operation. Results Compared with simple cerebral hemorrhage group, blood glucose increased significantly 1 and 6 h after operation in rats of 2 g/kg and 4 g/kg groups; ATP content significantly decreased and lactic acid content significantly increased in perihematoma brain tissue of rats in 2 g/kg and 4 g/kg groups (P<0.05). Conclusion Hyperglycemia after cerebral hemorrhage can aggravate energy metabolism disorder of brain tissue and brain damage. The degree of damage is related to blood glucose concentration.
