急性心肌梗死患者血小板膜糖蛋白动态变化的研究

摘    要:目的观察急性心肌梗死(AMI)患者血浆中血小板膜糖蛋白的动态变化,并与稳定型心绞痛(SAP)患者进行比较。方法采用酶联免疫法测定17例AMI5个时限、30例SAP、30例正常人的血小板α颗粒膜蛋白(GMP-140)和血小板膜糖蛋白Ⅱb/Ⅲa(GPⅡb/Ⅲa)。结果AMI组各时间的GMP-140与GPⅡb/Ⅲa,均高于对照组(P<0.01)及SAP组(P<0.01)。AMI组与SAP组比较,GMP-140、GPⅢb/Ⅲa在24h、48h、72h均有显著性差异(均P<0.01);1周、2周时无显著性差异(P>0.05)。AMI组自身的GMP-140和GPⅡb/Ⅲa有显著的动态变化(F=34.31,F=21.71均P<0.01);24h、48h与1周和2周比较均有显著性差异(均P<0.05)。24h与48h、72h相比,48h与72h相比,及第1周与第2周相比则均无显著性差异(均P>0.05)。结论;AMI时血小板功能明显活化,持续3天增高,提示血小板膜糖蛋白参与了AMI冠脉内血栓形成的过程。

Abstract:To study the dynamic changes of platelet membrane glycoprotein in AMI. Methods:Levels of serum platelet GMP-140 and GP Ilb/IIIa were measured in 17 AMI patients(24 h,48 h,72 h, 1 w,2 w) ,30 SAP patients,and 30 healthy persons using ELISA. Results:Levels of GMP-140 and GP Ilb/IIIa were significantly higher at each time point in patients with AMI compared with the control and SAP groups(P<0.01). There was a significant difference in GMP-140 and GP Ilb/IIIa between AMI and SAP groups at 24 h,48 h,and 72 h(P<0.01). There were significant dynamic changes in GMP-140 and GP Ilb/IIIa in AMI(P< 0.01). Conclusion:Platelet was activated for at least 3 days in AMI, which suggested that platelet membrane glycoprotein took part in the process of coronary thrombosis in AMI.
