柯萨奇病毒感染与过敏性紫癜的关系

摘    要:目的探讨过敏性紫癜(HSP)与柯萨奇病毒(CBV)感染的关系。方法 ELISA法检测118例HSP、60例肺炎和30 例健康儿童静脉血中CBV、呼吸道合胞病毒(RSV)、腺病毒(ADV)、EB病毒(EBV)和肺炎支原体(MP)等病原特异性IgM,速率法检测其血心肌酶谱。结果 1.HSP组病原感染阳性:CBV 58例(49.1%)、RSV 2例(1.7%)、ADV 1例(0.8%)、EBV 10例 (8.5%)、MP 3例(2.5%),CBV感染率与同期肺炎及健康儿童比较有显著性差异(P<0.05)。2.HSP皮疹反复组CBV-IgM阳性率与非反复发作组比较有显著差异(P<0.05)。3.CBV感染与无CBV感染患儿CK-MB、LDH、HBDH比较有显著性差异。4. CBV感染患儿中HSPN(HSP肾炎)组与非HSPN组心肌酶谱比较无显著差异。结论 CBV感染可能是HSP的重要诱发原因。 HSP反复发作和心肌酶谱异常与CBV感染有关。

Abstract:Objective To explore the relationship between the onset of Henoch - Schonlein purpura (HSP) and infection of Coxackie B virus (CBV) as well as the clinical features of HSP with CBV infection. Methods Profile of cardiac enzyme and the antiviral IgM of CBV, respiratory syncytial virus(RSV),EB virus (EBV).adenovirus(ADV) and mycoplasma pneumoniae(MP) in venous blood of children with HSP( 118 cases) or pneumonia (60 cases) were measured by enzyme - linked immunosorbent assay. Thirty health children were involved in this research as normal controls. Results 1. The percentages of antiviral IgM positive cases were 8.5 % (10 cases), 1.7 % (2 cases), 0.8%(1 case), and 2.5%(3 cases) of CBV, RSV, ADV, EBV and MP,respectively. There were significant diffe-rences of CBV-IgM positive rates between HSP children and pneumonia children and health children. 2. Forty- three HSP cases got reurrent rash. Among 43 cases, 27 cases were CBV- IgM positive, which were significantly higher than those of the CBV- IgM negative cases. 3. CK - MB, lactate dehydrogenase and hydrorybutyrate dehydrogenase were significantly higher of CBV- IgM positive cases than those of the CBV- IgM negative cases among HSP children. 4. There were no significant differences of cardiac enzyme between HSPN and HSP children without nephritic lesion. Conclusions CBV infection may be an important induce cause for HSP. Recurrent onset of rash and increase of cardiac enzyme among HSP children have close relationship with CBV infection.
