桥本甲状腺炎患者甲状腺功能及自身抗体状况

目的探讨桥本甲状腺炎患者血清甲状腺激素、高敏促甲状腺素（h-TSH）、甲状腺自身抗体浓度及相互关系。方法对明确诊断为桥本甲状腺炎的106例患者和正常对照组110例测定血清总甲状腺素（TT4）、游离甲状腺素（FT4）、总三碘甲状腺原氨酸（TT3）、游离三碘甲状腺原氨酸（FT3）、h-TSH、甲状腺过氧化物酶抗体（TPOAb）和甲状腺球蛋白抗体（TeAb），分析两组间各指标的差异以及患者组h-TSH和TPOAb、TgAb的关系。结果患者组TPOAb、TgAb和h-TSH显著高于对照组（P〈0．01），TT4显著低于对照组（P〈0．01），而FT4、TT3、FT3两组差异间无显著性。桥本甲状腺炎患者血清h-TSH浓度与TPOAb、TgAb相关分析显示，r分别为0．023和0．028，P分别为0．818和0．774。结论桥本甲状腺炎患者甲状腺功能较正常人群低，而h-TSH、TPOAb和TgAb则显著高于正常人群，提示桥本甲状腺炎发生发展过程中存在体液免疫作用。检测两自身抗体，尤其是TPOAb有利于普查发现桥本甲状腺炎。

Thyroid function and thyroid autoantibodies in patients with Hashimoto's thyroiditis

ObjectiveTo investigate the serum concentration of TT_4, FT_4, TT_3, FT_3, h-TSH, TPOAb and TgAb,and the relationship among h-TSH and TPOAb, TgAb in patients with Hashimoto's thyroiditis. Methods One hundred and six patients with Hashimoto's thyroiditis and 110 controls were recruited in this study. Serum TT_4, FT_4, TT_3 FT_3, h-TSH, TPOAb and TgAb were measured in the two groups. Results There were significant differences between the controls and the patients in serum concentration of h-TSH, TPOAb, TgAb and TT_4. Correlation analysis demonstrated that no significant correlation was found among the serum concentrations of h-TSH and TPOAb, TgAb (P=0.818, 0.774 respectively). Conclusion The patients with Hashimoto's thyroiditis had a lower serum level of TT_4, and higher level of h-TSH, TPOAb and TgAb. High concentration of two autoantibodies in patients with Hashimoto's thyroiditis suggested that humoral immunity maybe a critical progress in this disease. To measure serum level of TPOAb and TgAb can be helpful to screen Hashimoto's thyroiditis and to differentiate it from other thyroiditis.
