胆囊癌在CT不同增强时相内的显示差异性分析

目的结合胆囊癌的CT多期动态增强表现,分析胆囊癌在不同增强时相内病变显示的差异性。方法回顾性分析经手术病理证实的胆囊癌36例的CT征象。结果对于壁厚型胆囊癌,多期动态增强CT的不同时相内对胆囊癌的直接征象的显示情况差异有统计学意义（χ2=23.04,P〈0.05）。在多期动态增强CT的不同时相内对显示肿块型胆囊癌是否合并淋巴结转移组间比较差异有统计学意义（χ2=7.52,P〈0.05）。在多期动态增强CT的不同时相内对显示壁厚型胆囊癌是否合并淋巴结转移（χ2=13.14,P〈0.05）及肝门受累情况（χ2=24,P〈0.05）组间比较差异有统计学意义。在多期动态增强CT的不同时相内对显示腔内结节型胆囊癌是否合并淋巴结转移组间比较差异有统计学意义（χ2=6,P〈0.05）。在多期动态增强CT的不同时相内对显示胆囊颈型胆囊癌是否合并肝门受累组间比较差异有统计学意义（χ2=6,P〈0.05）。结论动态增强CT的不同时相内对胆囊癌的直接征象及间接征象的显示有差异,动态增强CT扫描对于胆囊癌的诊断、临床分期及治疗方案的制定很重要。

Analysis for the variability of CT findings at different enhancement phases for gallbladder carcinoma

Objective To analyze the difference of CT findings at different enhancement phases for gallbladder carcinoma according to the diffrent dynamic enhancement manifestations showed by CT.Methods The CT findings in 36 patients with gallbladder carcinoma identified by pathological examination after the operation were retrospectively analyzed.Results For the wall thickening type gallbladder carcinoma,there were significant differences in the CT findings of multiple phases dynamic enhancement at different phases（P〈0.05）.The CT findings of multiple phases dynamic enhancement at different phases could show whether the wall thickening type gallbladder carcinoma was combined with lymph node metastasis or not（P〈0.05）.For the wall thickening type gallbladder carcinoma,there were significant differences in the CT findings of multiple phases dynamic enhancement at different phases between gallbladder carcinoma combined with lymph node metastasis and hepatic hilum involvement（P〈0.05）.The differences in the CT findings of multiple phases dynamic enhancement at different phases were significant in showing whether intracavitary nodular type gallbladder carcinoma was combined with lymph node metastasisot not（P〈0.05）,There were significant differences in the CT findings of multiple phases dynamic enhancement at different phases between collum vesicae biliaris type gallbladder carcinoma combined with lymph node metastasisot and hepatic hilum involvement（P〈0.05）.Conclusion The direct signs and indirect signs of dynamic enhancement CT at different phases are different in showing the characteristics of gallbladder carcinoma,and dynamic enhancement CT scanning plays an important role in the diagnosis,treatment,clinical staging and the drawing up of treatment protocol.
