老年慢性阻塞性肺疾病患者应用无创呼吸机的疗效评价

目的：讨论无创呼吸机正压通气用于治疗慢性阻塞性肺病（COPD）并发呼吸衰竭的作用。方法：选择60例COPD并发呼吸衰竭的患者，随机分为 NIPPV 组（无创正压通气联合常规综合治疗）和对照组（常规综合治疗），在治疗前及治疗24h后观察临床症状、动脉血气分析。结果：NIPPV 组治疗后，呼吸困难，发绀等临床症状改善时间及程度明显优于对照组，PaO2较治疗前明显改善（P〈0.01），PaCO2与治疗前及对照组治疗后亦有明显下降（P〈0.05）。结论：无创正压机械通气具有无创伤性、早期使用并发症少、易被患者接受和使用方便等特点。该方法可以迅速缓解患者呼吸困难，改善呼吸肌疲劳，增加肺泡通气，提高PaO2，降低PaCO2。因而当COPD患者出现呼吸衰竭的呼吸肌疲劳问题时，应用无创正压机械通气治疗是一个行之有效的解决方法，值得临床广泛推广。

Clinical evaluation of non-invasive ventilation in the treatment of elderly patients with chronic obstructive pulmonary disease

Objective：To investigate clinical effects of non-invasive ventilation on elderly patients with COPD. Methods：60 cases patients were divided into the control group （given conventional therapy） and the treatment group （given non-invasive ventilation）, the treatment effects of two groups were compared. Result：The levels of PaCO2 and PaO2 in the treatment group were better than that in the control group,the difference was statistically significant （P〈0.05）. Conclusion：The efficacy of non-invasive ventilation on treating COPD in elderly patients was significant,and worthy of clinical promotion.
