后腹腔镜下与开放手术治疗局限性肾癌的比较

目的行后腹腔镜下肾癌根治术与开放性肾癌根治术总结比较。方法分析后腹腔镜下肾癌根治术（治疗组）28例、开放性肾癌根治术（对照组）30例临床资料,总结治疗效果。结果两组病例年龄、性别、肿瘤大小、分期、手术时间、生存率无明显差异。术中出血量、住院时间、术后进食时间、术后恢复正常活动时间、术后下床时间、肠功能恢复时间、术后并发症等方面,治疗组优于对照组,差异有统计学意义（P〈0.05）。结论后腹腔镜下肾癌根治术较开放性肾癌根治术具有安全性高、创伤小、术后恢复快、并发症少等优点,临床应用前景广泛。

Retroperitoneal laparoscopy vs open approach to nephrectomy in localized renal cell carcinoma

Objective Retroperitoneal laparoscopy in comparison with open approach to nephrectomy in localized renal cell carcinoma. Methods 28 cases by retroperitoneal laparoscopic radical nephrectomy（treatment group） and 30 cases by open retroperitoneal laparoscopic （control group）. We reviewed the clinical data and oncologic outcome. Results There is no statistically significant difference in the 2 groups＇ age, sex, tumor size, tumor staging, operation time and survival rates. Blood loss during operation, the hospitalization time, eating time after operation, normal recovery time, get out of bed time and postoperative complication, and the time of intestinal function recovery were analyzed and compared. The datum of treatment group is better than those of control group and has statistically significant. Conclusion Compared with open radical nephreetomy, the retroperitoneal laparoscopy radical nephrectomy is higher safety, less trauma, and lower narcotic requirement and complications. There is good prospect in clinical application of retroperitoneal laparoscopy radical nephrectomy.
