复合小梁切除术治疗原发性青光眼

目的探讨复合小梁切除术（小梁切除联合丝裂霉素C及可调节缝线）治疗原发性青光眼的效果。方法将61例（78只眼）原发性青光眼随机分为观察与对照两组，分别行复合小梁切除和单纯小梁切除术，同期进行疗效观察与对比，指标包括眼压、滤过泡、前房深度、视力及其他并发症，随访3～14个月。结果术后早期，观察组浅前房及其他并发症的发生率明显低于对照组，而视力及眼压两组差异无统计学意义（P〈0．01）；术后远期观察组眼压明显低于对照组，而视力下降及非功能性滤过泡的发生率观察组明显低于对照组。结论复合小梁切除术可有效地防止术后早期的低眼压、浅前房及后期的滤过泡疤痕化，从而获得持续、稳定、正常的眼压，值得在抗青光眼术中推广。

Trabeculectomy combined with mitomycin C and controlled suture in primary glaucoma

Objective To study the clinical effect of trabeculectomy combined with mitomycin C and controlled suture in primary glaucoma.Methods Sixty-one cases(78 eyes) with primary glaucoma were randomized into two groups:the observed group underwent trabeculectomy combined with mitomycin C and controlled suture;the controlled group with traditional trabeculectomy.The intraocular pressure(IOP),filtering blebs,the anterior chamber depth,the visual acuity and other complications were measured and compared between the two groups during 3-14 months after operation.Results In the early stage of postoperation,the incidence of postoperative shallow anterior chamber and other complications in the observed group were less than those in the controlled group,and the IOP and visual acuity of the two groups had no difference.In the late stage of postoperation,the IOP and the incidence of visual acuity loss of the observed group were less than those of the controlled group.Conclusion Trabeculectomy combined with mitomycin C and controlled suture can effectively reduce the incidence of shallow anterior chamber and the scaring of filtering blebs,it can therefore make the eyes get constant normal IOP.It is worthy of application in glaucoma surgical operation.
