全髋关节置换与股骨近端髓内钉治疗高龄股骨粗隆间骨折的临床疗效对比研究

目的比较全髋关节置换和股骨近端髓内钉两种手术方式治疗老年性股骨粗隆间骨折的疗效,评价二者的优劣。方法回顾性分析2009年6月～2012年5月我院收治的老年股骨粗隆间骨折患者66例,分别采用THA与PFN治疗,比较两组患者的手术创伤、手术时间及功能恢复等指标。结果采用髋关节功能Harris评分系统评定,两种手术方法均获得良好的临床疗效。在切口长度、手术时间和术中出血量方面,PFN优于THA组（P〈0.05）;而术后卧床时间及内科并发症方面,THA组更优（P〈0.05）。结论 THA与PFN均为治疗老年性股骨粗隆间骨折的有效方法,临床上应根据患者骨折特点来选择最合适的手术方式。

Curative Effect Comparison between Total Hip Arthroplasty and Proximal Femur Nail in Treating Senile Patients with Intertrochanteric Fracture

Objective To compare the curative effect of total hip arthroplasty （THA） with that ot proximal femur nail（PFN）in treating senile patients with intertrochanteric fracture. Methodr A retrospective analysis was made to the clinical data of 66 senile cases with intertrohanteric fracture; according to the patients＇ will, 66 cases were divided into 2 groups： THA group （n=26） and PFN group （n=40）; the surgical wound, operation time length and function recovery of the patients in the 2 groups were compared. Results Both THA and PFN were of good clinical curative effect according to Harris hip function score system; PFN group was superior to THA group in incision length, operation time and bleeding volume in operation （P〈0.05） while THA group was superior in bed rest time after operation and complications to PFN group （P〈0.05）. Conclusions Both THA and PFN are effective in the treatment of the senile pa- tients with intertrochanteric fracture and the choice should be made based on the fracture situation of the patients.
