高血糖与胆石症40例临床报告

目的 分析胆石症和糖尿病发病率之间的关系。讨论糖尿病合并无症状胆石症时是应该手术治疗以及糖尿病病人胆道手术的围手术期处理。方法 对４０例合并高血糖的胆石症病人进行回顾性分析。结果 外科胆石症 ６９３例，其中４０例合并高血糖。男性１４例，女性２６例，平均年龄６０．１岁。确认糖尿病１５例，可疑糖尿病１１例，应激性高血糖１４例。手术治疗３３例，胆囊切除１４例，胆囊切除、胆总管探查１６例，胆肠内引流２例，

Gallstones in diabetes

Objective To analyse the association of gallstones with diabetes, and to discuss the prophylactic removal of asymptomatic gallstones and the perioperative management of biliary surgery in diabetic patients.Methods Forty cases of hyperglycemia in gallstone patients were reviewed retrospectively.Results There were 40 hyperglycemia cases among 693 gallstone patients. Diabetic mellitus was diagnosed in 15 patients, and was suspected in 11 patients; stress hyperglycemia was met in other 14 patients. Operations were performed on 33 cases. Postoperative complications included wound infection in 2 ,pneumonia in 3, urinary infection in 2.Two patients were complicated with acute edematous pancreatitis. One woman died from coexisted acute obstructive suppurative cholangitis, coronary heart disease, and severe atrio ventricular block.Conclusion Diabetes is frequently associated with gallbladder disaese. Acute cholecystitis with diabetes may have serious complications. Diabetic patients with asymptomatic gallstones should have their gallbladder and gallstones removed at least once the gallstones become symptomatic.
