颈动脉狭窄血管内支架治疗8例

摘    要:目的:总结颈动脉狭窄血管内治疗的基本方法,探讨该术式治疗颈动脉狭窄的临床意义,并发症及其治疗经验。方法:对8例颈动脉狭窄患者进行血管内治疗,对全部患者进行全脑血管造影术及围手术期抗凝治疗,4例患者使用脑保护装置,共放支架8个,其中Wallstent支架3个,smart支架5个。结果:1例术中出现微栓子脱落轻度卒中,1例术中出现一过性心动过缓血压下降,其后逐渐恢复,I例术后持续性低血压,7例患者症状好转或消失,占87.5%,在使用脑保护装置的4例患者中均无神经系统并发症。对8例患者进行随诊,随诊时间1年半无明显并发症。结论:颈动脉狭窄血管内支架治疗是一项安全有效的治疗手段,采用正规的围手术期治疗和应用脑保护装置可以减少神经系统并发症。

Abstract:Objective: To discuss the procedure of carotid stenting for internal carotid artery. To study the clinical significance of carotid stenting for internal carotid artery. Methods: 8 patients with ICA stenosis were included from October 2004 to October 2005 . All of them took periprocedual anticoagulation treatment and cerebral vascular angiography .4 patients used cerebral protection of filter devices. 8 stents were implanted .Three of these stents were Wallstent and five were Smart stents. Result: Amomg them dislocation of microemboli and small stroke happened in 1 patient. The heart rates decreased and the blood pressure declined in 1 patient and had a recovery after salvage . 1 patient had a long-time decline of blood pressure . 7 patients (87.5%) turned for the better. 4 patients who received cerebral protection device did not have nervous complication. 8 patients(100%) were followed up in one year and a half, they all have not nervous system complications . Conclusion: Carotid stenting is effective and safe for the treatment of carotid artery stenosis . If proper periprocedual treatment are taken and cerebral protection device are used , it could decrease the nervous system complications.
