外固定支架行损伤控制性治疗在创伤骨科修复中的应用体会

目的探讨外固定支架行损伤控制性治疗在创伤骨科修复中的应用及效果。方法选用经损伤控制性治疗和未进行此项治疗的重型骨盆骨折患者各45例进行分析。应用损伤控制性治疗的患者作为观察组，未采用的患者作为对照组。观察组先行一期外固定支架治疗，一般情况稳定后采用二期切开复位治疗。对照组采用一期切开复位内固定治疗。结果观察组中存活39例，死亡6例。对照组中存活30例，死亡15例。观察组患者的术中失血量、手术时间、术后体温恢复时间、乳酸清除时间、PT／APFT恢复时间明显少于对照组（尸〈O．05）。结论DCS治疗效果明显、预后好，值得应用。

Experiences on external fixation - the line of damage control orthopedic therapy in the repair of trauma

Objective To investigate the application and effects of external fixator-damage control therapy in rehabilitation of orthopedic trauma.Methods 45 cases with severe pelvic fracture were given damage control therapy as observation group. Other 45 patients were selected as control without being given damage control therapy. In the observation group, external fixation was firstly used, and then, after conditions being stable, second stage open reduction treatment was also conducted. As for the patients in control group, only the first stage open reduction was carried out.Results Among the observation group,39 patients survived, 6 died. 30 patients survived and 15 cases died in the control group. The intraoperative blood loss, surgery time, recovery time of the body temperature, lactic acid clearance time, PT / APTT recovery time in observation group were significantly less than those of the control group (P＜0.05). Conclusion The curative effects of damage control treatment are obvious with good prognosis, and worth being used popularly.
