胃术后胃肠减压的探讨

目的：寻找胃手术后合理胃肠减压的指征及终止时机。方法：对我院１９９０－１９９８年间５９７例胃手术后胃肠减压的情况进行了分析、总结。结果：２２８例采用了胃肠减压，３６９例未采用胃肠减压而无消化道瘘发生；胃肠减压终止时间与肛敢时间并不完全一致。结论：胃手术后一律行胃肠减压的作法不可取，应选择易发生穿孔再发、胃空障碍、吻合口或十二指肠残端瘘的讽顾病例作为胃肠减压的对象；终止胃肠减压的时机仍以肛门排气为主

Study on postoperative gastrointestinal decompression.

Objective:To find out indication and termination time given the rational gastrointestinal decompression after operation on the stomach.Methods:597 patients who were given postoperative gastrointestinal decompression in our hospital have been analyzed.Results:Among them,228 patients have been given gastrointestianal decompression.But other 369 patients who have not been given the decompression have no symptoms in digestive tract leakage or fistula.The termination time of gastrointestinal decompression is not completely consistent with the exsufflation time of anus.Conclusion:Therefore,gastrointestinal decompression should be selectively used.The high risk factors,such as perforating relapse after the operation,gastric emptying disturbance,anastomotic stoma or stump leakage of duodenum,can be taken as the indication.It is the suitable time to end gastrointestinal decompression when the anus exsufflates.The ending time can be moved up or delayed.
