| 摘 要： | 我院通过电话随访、家庭访视、电话咨询、造口专科门诊的定期复诊、造口人阳光俱乐部活动等对44例永久性造口患者实施出院后延续护理。44例患者均按计划完成各项出院后延续护理干预,效果满意,出院后1个月、6个月时患者的造口知识掌握情况优于出院时(P<0.01),出院后3个月时自我护理能力高于出院时(P<0.01)。

| Abstract: | This paper summarized the application and effect of transitional care in 44 discharged patients with a permanent colostomy. Transitional care included telephone follow-up, necessary home visit, telephone consultation initiated by patients, routine subsequent visit in stoma clinic, and ostomates＇ club activities etc. Forty-four patients completed all of the transitional care as planned and the results were satisfactory. The stoma-related knowledge of 1 month and 6 month discharged colostomy patients were significantly better than those of 1 day before discharge （P〈0.01）. The self-care ability of 3 month discharged colostomy patients were significantly better than those of 1 day before discharge （P〈0.01）. |
