应变率成像技术对风湿性心脏病右室收缩功能的研究

摘    要:目的探讨应变率成像技术在评估风湿性左心瓣膜病患者右室局部心肌收缩功能方面的价值。方法将风湿性左心瓣膜病组按三尖瓣返流速度估测肺动脉高压的程度,选取其中的肺动脉高压轻度组作为研究对象,获取正常组和风湿性左心瓣膜病组右室游离壁及室间隔右窒面心肌的基底段、中间段及心尖段的应变、应变率曲线,并且分别测量每一曲线上的收缩期峰值应变和应变率。结果风湿性左心瓣膜病肺动脉高压轻度组,其(1)右室游离壁及室间隔右室面收缩期峰值应变均小于正常组(P〈0.05);(2)右室游离壁基底段、中间段、心尖段和室间隔右室面中间段的收缩期峰值应变率均小于正常组(P〈0.05)。结论应变率成像技术是评价风湿性左心瓣膜病右室收缩功能的一种敏感有效的检查方法,可以在病变早期发现右室局部心肌收缩功能的改变。

Abstract:Objective To explore the values of strain and strain rate techniques for evaluating regional myocardial systolic functions of right ventricular walls in rheumatic cor sinistrum valve Disease. Methods The article' s study objects were the rheumatic cor sinistrum valve disease group of low level pulmonary hypertension(PH) which estimated by right atrioventricular valve backstreaming speed. The curves of strain and strain rate were measured in subendocardial layers of basal, mid and apical segments of the free walls of right ventriculars and interventricular septums. The peak systolic strain(PST) and peak systolic strain rate(PSTR)were respectively measured in each curves. Results In rheumatic cor sinistrum valve disease group, (1)the means of PST in all segments of right ventricular walls were less than those in the normal group (P〈0.05), (2)the means of PSTR in basal,mid and apical segments of right ventricular free walls and in the mid segments of interventricular septums were less than those in the normal group(P〈0.05). Conclusion Strain rate techniques is a sensitive and available detection method to evaluation of right ventricular systolic functions in rheumatic cot sinistrum valve disease, which can detect the change of regional myocardial systolic functions.of right ventricular walls in earlier period.
