老年高血压合并腔隙性脑梗塞患者胰岛素抵抗的临床研究

目的 :研究胰岛素抵抗 (IR)与高血压合并腔隙性脑梗塞患者之间的关系。方法 :选取 2 8例高血压合并腔隙性脑梗塞患者作为研究对象 ,测定血糖(FPG)、胰岛素水平 (FINS)、C 肽、总胆固醇 (CH)、甘油三酯 (TG)。另 2 8例正常健康老年人作为对照组。同时计算胰岛素敏感指数 (ISI)。结果 :高血压合并腔隙性脑梗塞患者FINS、C 肽水平显著高于对照组 (P <0 .0 1) ,其ISI较对照组显著降低 (P <0 .0 1)、脑梗死组FINS显著高于正常对照组 (P <0 .0 5) ,其ISI较正常对照组显著降低 (P <0 .0 5)。结论 :老年高血压合并腔隙性脑梗塞患者存在IR ,IR作为腔隙性脑便塞独立危险因素应引起高度重视。

Clinical Study on the Relationship Between Insulin Resistance and Elderly Hypertension Patients with Lacunar Celebral Infarction

Objective:To sutdy the relationship between insulin resistance( IR) and lacunar cerebral infarction with elderly hypertension patients.Method:56 patients were divided into two groups,28 cases elderl y hypertension patients with celebral lacunar infarction,the other 28 cases nor mal control.Their fasting plasma glucoses,serum c-Ps,insluin levels,chol esteral and triglyceride before breakfast were measured.Their insulin sensitivit y indexes(ISI) were compared each other.Results:Insluin、C-Ps are signsificantly higher and ISI is sig nificantly lower in patients with cerebral lacunar infarction than that of norma l cont rols(P<0.01).Conclusion:IR exists in cerebral lacunar in farction with elderly hypertension patients and it is an important risk factor f or cerebral lacunar infarction.
