经皮下空肠盲襻胆道取石术的应用体会（附11例报告）

采用经皮穿刺皮下空肠盲襻置管引流减压、胆道造影及皮下空肠盲襻切开胆道取石术治疗胆道残余结石或复发性结石并急性重症胆管炎１１例。结果：穿刺胆道引流减压后全组病例临床症状立即缓解。胆道造影显示胆总管下端结石嵌顿３例，胆总管多发结石５例，胆总管及肝内胆管结石３例。其中１０例取净结石，治愈率９１％（１０／１１）；１例改行经腹胆道切开取石及胆肠Ｒｏｕｘ－ｅｎ－ｙ吻合，经腹手术率９％（１／１１）。并对胆道取石方法和手术的特点进行了讨论。

Application of Subcutaneous Jejunal BIind-Loop Incision for Choledocholithotomy(A Report of 11 Cases)

Eleven patients suffering from retained or recurred bileduct stones accompanied by acuteobstructive supperative cholangitis(AOSC)have been treated by percutaneous subcutaneousjejunal blind-loop catheterization for drainage of biliary tract and choledocholithotomy. TheresuIts indicated that clinical symptoms and signs were alleviated immediately in all cases af-ter percutaneous bileduct drainage. Cholangiography showed 3 patients with calculi impactedin the lower part of the common bile duct,5 with multi-choledocholith and 3 with stones inthe common bile duct and intrahepatic bile ducts. Calculi in 10 of the 11 cases were with-drawn thoroughoy. One case underwent cholelithotomy and Roux-en-Y choledochoje-junostomy through abdominal operation. The method of choledolithotomy and characteristicsof the operation have been discussed.
